Fully Endoscopic Resections of Vestibular
نویسنده
چکیده
▼ Background: We report a series of 527 patients with unilateral vestibular schwannomas (VS) who underwent fully endoscopic resection of their tumors during the period of October, 2001 to July, 2010. Patients ’ outcomes were evaluated, with specifi c regard to hearing preservation, facial nerve function, postoperative complications and completeness of the resection. Methods: The patient population consisted of 527 cases with unilateral VS(s); patients with neurofi bromatosis type 2 (NFT2) were excluded from this study. Tumors ranged in size from 0.3 – 5.8 cm, most tumors were less than 4 cm in diameter (mean: 2.8 cm). Tumors were removed via 2.0 cm “ keyhole ” retrosigmoid craniotomies. Results: Utilizing the fully endoscopic technique, 94 % of tumors were completely removed; subtotal removal was performed in 6 % of patients in an attempt to preserve their hearing. Anatomic preservation of the facial nerve was achieved in all of the patients. Functionally, measurable hearing (serviceable / some) was preserved in 57 % of cases that had either “ serviceable ” or “ some ” hearing pre-operatively. There were no major neurological complications such as quadriparesis, hemiparesis, bacterial or aseptic meningitis, permanent lower cranial nerve defi cits, or deaths. Conclusion: From our experience, we conclude that the endoscope is ideally suited for a minimally invasive approach for resection of vestibular schwannomas. D ow nl oa de d by : K es av a R ed dy . متن کاملCurrent Status of Endoscopic Papillectomy for Ampullary Tumors
Detection of tumors of the ampulla of Vater, including ampullary adenoma, has been improved by routine screening endoscopic procedures and imaging modalities. Endoscopic resection by endoscopic papillectomy is rapidly replacing classic surgical resection and is a less invasive procedure. Endoscopic resection can have a role not only in the final histopathologic diagnosis but also as a definite ...
متن کاملA self-assembling matrix-forming gel can be easily and safely applied to prevent delayed bleeding after endoscopic resections.
BACKGROUND Endoscopic resections have low morbidity and mortality. Delayed bleeding has been reported in approximately 1 - 15 % of cases, increasing with antiplatelet/anticoagulant therapy or portal hypertension. A self-assembling peptide (SAP) forming a gel could protect the mucosal defect during early healing. This retrospective trial aimed to assess the safety and efficacy of SAP in preventi...
متن کاملFully Endoscopic Resection of Intra-Axial Brain Lesions Using Neuronavigated Pediatric Anoscope.
AIM To present a new simple disposable tubular retractor which provides fully endoscopic resections of the intraaxial brain lesions. MATERIAL AND METHODS A total of 13 patients underwent surgical resection of an intraaxial brain lesion larger than 3 cm with a fully endoscopic approach using the navigation-guided pediatric anoscope. The anoscope was adapted to serve as a tubular retractor. All...
متن کاملCurrent status of laparoscopic therapy of colorectal cancer.
Laparoscopic colorectal resections offer several benefits postoperatively, including minimal impairment of gastrointestinal and pulmonary function, less immunosuppression, shorter hospital stay and improved reconvalescence. Since the introduction of laparoscopic surgery for the therapy of curable colorectal cancer, some concern was voiced in terms of oncologic radicality, the issue of port-site...
